Making a Referral
There are several ways in which you can be referred to Headway Basingstoke. These referrals can be made by a healthcare professional or you can self-refer.
We accept referrals for adults with a confirmed diagnosis of acquired brain injury resulting from causes including stroke, brain tumour, encephalitis and traumatic brain injury. The person must be over 18 however, and their injury will have occurred in adulthood. Confirmation of brain injury will be sought from medical professionals working with the individual in cases of self-referral or referrals from a non-clinician – this will take additional time.
The easiest way to refer is by downloading and completing the Referral Form. Provide as much information as possible to enable the Headway team to make informed decisions and effective assessment of needs. The completed form should be emailed back to the Referrals Administrator or you can post it to us at the address at the bottom of the page.
Our Carers Services are open to anyone providing care and or support to someone with a confirmed brain injury living in the Basingstoke and North Hampshire area. This may include spouses, partners, parents, siblings, other family members or close friends. If you are a carer and looking for information or support for yourself, then visit our Carers section to find details of our services for Carers.
